Your First Visit

• On your first visit, we will perform a consultation and physical exam to determine the source of pain. Please allow approximately 45 – 60 minutes, so that we may perform any necessary exams and begin therapy.
• We recommend arriving 15 – 20 minutes prior to your scheduled appointment time to complete any registration forms and verify your insurance benefits.
• If possible wear comfortable clothing, such as shorts and t-shirt or workout clothing which allows for free movement
• Please bring the following items with you on your initial visit to help assist the doctor with your care.
  • Imaging / Test / Physician Records that you suspect may be applicable
  • List of current medication(s)
  • Insurance Card
  • Photo ID
  • Co-payment, co-insurance or deductible if applicable
  • Any other paperwork or information that pertains to your treatment or
    condition

Insurance and Payment Information

We accept most insurance plans, including Medicare, and handle worker’s compensation and accident-related claims.

Insurance: We are a contracted provider for many insurance plans. If you have questions about using your insurance at this facility, please ask about coverage prior to the start of your examination or treatment. Our staff is happy to verify and explain your chiropractic benefits, prior to treatment. Your insurance provider will determine your actual benefits and coverages. We encourage you to also contact your plan administrator to verify your specific chiropractic benefits, pre-notification or referral requirements, because not all charges are always covered.

No Insurance: If you have little or no insurance, you can still receive affordable care. Middle Tennessee Chiropractic and Sports Injury has discounted rates available through the ChiroHealthUSA (CHUSA) program. Signing up is easy and only takes a few minutes of your time when you come into the office. The CHUSA member discount rate is 25% off all clinic professional services along with capped fees for visits, and a 10% discount on durable medical goods and nutritional supplements.
